Haven’t found the right pet yet? Send us a general application and we’ll keep you in mind as animals come in — you can always apply for a specific pet later.
Adopters must be 21 or older.
House number and street — city and state are above.
If you rent. We may call to confirm pets are allowed.
Adults, children and their ages.
Privacy, chain link or other — and roughly how tall.
Your work schedule, time spent away from home, and how you plan to fit exercise, mental stimulation and enrichment into your dog's day.
Please describe the activities you plan to provide.
For example a running or hiking companion, an active playmate, a dog for training or dog sports, or a more relaxed companion.
For each pet: name, species, breed, age, and whether they are spayed or neutered. Write “none” if you have no pets.
We may call to confirm your pets' care.
If yes, please explain in the box below.
If you have one in mind — leave blank to be matched as animals come in.
My dog will live primarily indoors as part of my family. I will provide fresh food, water, supervision, exercise and safe housing every day. I will provide regular veterinary care at a licensed clinic, and emergency care when needed. I understand I am financially responsible for veterinary costs after adoption. I understand dog ownership is a long-term time and financial commitment. I understand Frankie's Friends has policies on rehoming, transfer and surrender, set out in the adoption contract.
This is state law. If you do not agree, please do not continue with this application — the rescue will collect the animal if proof of sterilization is not shown.
We check it in person rather than asking you to type it here.
The person who would take full-time care of your pet if you could not, temporarily or permanently. Frankie's Friends will contact them to verify.
Today's date is recorded with your application.
We’re a volunteer-run rescue. We read every application and usually reply within two days. You’ll get an email confirming this arrived.
